DOR Nonemployee Confidentiality and Disclosure Statement (COM 103)
MVTRIP Effective September 5, 2012, all MVTRIP users with access to motor vehicle data will be required to complete a new COM 103. At login, the user will be prompted to complete the COM 103 form. Once completed, the COM 103 form will be electronically submitted to the DOR computer security office. Please contact the computer security office if you have any questions regarding this notice.
